  • Approved Continuing Education / Recertification Classes

    Only courses on this list have been approved by WVDEP for UST Worker continuing education. Course Certificates MUST contain the course title, number of hours of continuing education credit as approved by WVDEP, the WVDEP provided course number, participant’s name and UST certification number(s), and the date and location of the training. If the certificates do not contain all of the above-mentioned information, the course will not be accepted for WVDEP UST Worker continuing education.

  • Operating and Maintaining Underground Storage Tank Systems

    This booklet is for owners and operators of underground storage tank (UST) systems regulated under 40 CFR part 280. You are responsible for making sure your USTs do not leak. This booklet can help you meet your UST responsibilities.

  • Interstitial Monitoring Testing Report Form

    The applicable portions of this form must be completed and provided to the facility owner/operator within 30 days of the test date, along with written test procedures, data collection logs, and printouts from test equipment (if applicable). Facility must keep this page along with pages 2-3 as applicable on record for submittal to the WVDEP upon request. Each page must identify the facility and contain the WVDEP certified worker’s signature. WVDEP certified worker must be on site during all testing.

  • ATG Testing Report Form

  • UST Walkthrough Inspections Checklist

    Initial each box to indicate the equipment was inspected, as described. Use NA if the equipment inspection does not apply to the site. Take action for any alarms, damaged equipment and non-normal operating conditions; note actions taken on page two. Immediately report all fails of any monitoring equipment to WVDEP.

  • Notification for Underground Storage Tanks

    The primary purpose of this notification form is to provide information about the installation, existence, changes to, and closure of underground storage tank systems (USTs) that store or have stored petroleum or hazardous substances.

  • EPA's Investigation of Corrosion-influencing Factors in USTs with Diesel Services

    This research focused on better understanding a type of rapid and severe corrosion of metal components in underground storage tanks (USTs) storing diesel fuel. UST owners first began reporting this corrosion to UST industry servicing companies in 2007.

  • Cathodic Protection Test Reporting Form - Galvanic

    This form must be utilized to evaluate underground storage tank (UST) galvanic cathodic protection (CP) systems in the State of West Virginia. A copy of this completed form must be submitted by the owner /operator to the WVDEP UST Section within 30 days of testing. A site drawing depicting the UST cathodic protection system and all reference electrode placements must be completed.

  • Cathodic Protection Test Reporting Form - Impressed Current

    This form must be utilized to evaluate underground storage tank (UST) impressed current cathodic protection systems in the State of West Virginia. A copy of this completed form must be kept available & submitted by the owner /operator to the WVDEP UST Section upon request. A site drawing depicting the UST cathodic protection system and all reference electrode placements must be completed.

  • Operator Training Course Application

    Beginning August 8, 2012, all persons who operate, maintain, or are responsible for addressing emergencies presented by spills or releases from Underground Storage Tanks (USTs) must receive training based on their duties & level of responsibility for USTs, in accordance with Article 33 Section 30.7 of the WV Legislative Rules.

  • Insurance Industry Change Memorandum, dated June 18, 2012

    To ensure they are buying and retaining appropriate coverage for their UST systems, owners and operators should understand and be attentive to the underlying language, terms, and conditions of their policies. One primary purpose of state and federal regulation is to set forth specific requirements for underground storage tank to prevent product releases.
