EPA recently revised the Hazardous Waste Manifest Forms used to track hazardous waste from a generator's site to
the site of its disposition. Handlers of waste must obtain forms from a source that has registered with US EPA
to print and distribute the form. Please refer to EPA's Manifest Registry web page for a list of approved
registered printers. If you need additional information about the use of the manifest form for tracking
hazardous waste, please refer to US EPA's Hazardous Waste Manifest System
You no longer need to mail a coyp to the WV DEP.
